To answer your question, I live in Houston.  I did go see my endo today and , as with so many physicians that I've seen, she acted as though I were some kind of hypochondriac looking for something to be wrong. 

Despite having a normal ACTH Stim test, she ordered a 17 hydroxyprogesterone test.  I asked he why when the ACTH was normal and she said something to the effect of late onset CAH not being salt wasting, whatever that had to do with it.  She also told me that my inability to snap back from physical stressors like surgery is not an adrenal problem (but her expression read,  "you need to be in a padded cell".)   She also ordered testosterone and DHEA, which have been high in the past. 

I'm supposed to go back to her in two weeks, but I don't have very high expectations.   I'm already looking for alternatives.
