I also have late onset CAH.  When I became pregnant last year, my doctor started me on dexamethasone 1 mg per day (normally I take 0.25mg).  The rationale for the higher dose and the choice of dexamethasone was that if the baby was a girl with CAH, the dexamethasone would cross the placenta and suppress her adrenal gland to prevent virilization.  I ended up delivering twin girls and both were healthy without any signs of virilization.  But this was just how my doctor approached my situation.  Perhaps there is more than one approach.
