I've dealt with my share of labs, but I can't seem to fit these together. I tried to look through old posts to apply them, but I must be slow because I still don't understand. Can someone help?My baselines were:
DHEA unconjugated 204 ng/dl (marked as normal)
17 hydroxyprogesterone 81 ng/dl (marked as normal)
17 hydroxypregnenolone 72 ng/dl (mafked as normal)
prenenolone 21 ng/dl (marked as normal)
progesterone 0.5 ng/ml (marked as normal)
AM Cortisol 10.6 mcg/dl(marked as normal)
DHEA sulfate 97 mcg/dl(marked as normal)
Testosterone total 132 ng/dl (high, 2-45 reference)
Testerone free % 2.05 % (high, 0.5-2.0 reference)
Free testosterone 27.1 pg/ml (high, 0.1-6.4 reference)
Is this suggestive of CAH? I thought my cortisol or 17 hydroxy would be off. Based on this, endo did an ACTH stimulation test. This is really hard for me to interpret.....
Baseline at 30min at 60 min
Progesterone 0.47 hg/ml 0.85 0.92
ACTH 11 pg/ml <10 <10
Androstenedione 1.9 ng/ml 2.4 High 2.5 High
DHEA-S 107 ug/dl 118 125
DHEA 416 ng/ml 1012 High 1112 (normal?)
17 OH-Progesterone 1.8 ng/ml 4.8 5.8
(notes on above say postACTH < or = 3. CAH>15 heterozygotes 3-10)
Pregnenolone 32 ng/dl 185 High 186 High
17 OH-pregnenolone 49 ng/dl Low 1190 High 1190 High
*range for above reads 53-357
So is this diagnostic for CAH? Which kind?
Then Endo started me on Dexamethasone 0.5 and at first I was feeling good. Then started to get real moody and my hair was falling out and I just didn't feel good... so after a lot of complaining she ordered some labs they were:
Free testosterone (direct) 1.9 pg/ml Normal
DHEA Sulfate 26 ug/dl Low (65-380 reference)
So she cut my dosage of Dex in half to 0.25 mg daily.
Now I'm having this never ending 10 wk period so she wants to bump it back up to 0.5mg since she thinks the period problems began when she decreased the dose. Does this sound right, because I'm scared to take that much again???
HElp!
