Natalie, from what I understand, the 9th day of your cycle should be ok for checking 17 OHP, but the levels do drop during the day. I can't tell you how significant that drop is, however, but the recommendation is to have it checked before 9 am. So it is hard to tell how high your level would be if it was checked in the morning.

That being said, I'm sure you know that 0.75 mg is a high dose for LOCAH. But I think it's great that your endo responds to how you FEEL, rather than just the labs. He should take into account other labs than 17 OHP, however, like testosterone, DHEAS, and androstenedione (sp?). Those would help explain the oily skin, perhaps. There are other antiandrogens besides spiro, I think, so if the dex doesn't do a good enough job, you can try something else. I do believe that I would try to go to a lower dose of dex, if possible, though.
