You will need to speak to the surgeon and make sure they are aware of the need for cortisol and "above normal" monitoring. I have not had any surgery myself, but I believe a large cortisol dose is often given before the start of the operation.

As always, you are best to liase with your endo for advice, they are in the best position to help you.
