I don't know about the swelling, but the shakiness scares me a little. How often do you give her her meds? All of the endos we've had (4 now) say that CAH kids - especially the little ones - should get their meds on regular timed intervals. I know that there are still some endos out there that say not to get up in the middle of the night to give meds, but that just doesn't sit well with me, and we have always given meds in the night. In any case, it sounds to me like perhaps her blood-sugar could be low (this could be dangerous and happened to my daughter once when she was about 3) or her meds levels drop too low at night?? I would ask my endo and have them check this out; if her levels are, in fact, too low at night, adding a fever or bad cold into the mix might result in crisis.

I'm just speculating of course, but would get a (and perhaps second?) endo's opinion.
