I talked with my OB prior to becoming pregnant just as you did.  I called and told her as soon as I knew I was pregnant.  I was probably about 4 weeks, and I started on dex. three times a day.  I went in to see her much more often then I did with my son.  Also, we had to go in for genetic counseling to find exactly what gene and all was involved.  They said we had to have that done in order for the doctors to identify if the baby had CAH.  Then I had amnio at 15 weeks.  It took about a month for the results to come back..however I did fine out in a couple of days the sex.  It was a girl so I had to continue the dex. until the other results came in.  Finally they came in and she had CAH.  I will tell you I cried, and my heart was broken momentarily, but then I realized I have a wonderful son with CAH, so what was I so upset about.  I did have to continue the dex for the entire pregnancy.  It didn't make me gain alot of weight.  I didn't notice more of an appetite, but I did retain water big time.  I also got lots of stretch marks, and I didn't get any with my son.  I read that the dex thins your skin causing those.  I had no health problems from the dex.  My baby was born with normal genitals, so the dex. was definitely worth taking.  Like you I am about 4 hours from our endo.  My regular ped. was in contact with him, and they developed a plan.  She stayed for 1 week at the hospital.  However, they didn't start her on salt right away and we ended up in the hospital with our endo for a weekend when she was 2 weeks old.
