Hi Julia, I have read your previous posts about the positive outcomes of the circadian rythm dosing for your child. My 5-year old daughter has been under good control, (bone age matches her chronological age, she follows the growth percentile, she should be on, etc.) Our main concern was, that her clitoris started to grow, and I smelled under-arm body odour. So, instead of increasing her dose I suggested our endo to play around with the timing. For 5 years she took Cortef 3 times: 8-9 AM (right after she woke up), 12:30 (kindergarden) 10:00 (before goinmg to bed). First we tried taking meds 4 times a day, which seemed to work (8-9 AM) 12:30, 17:00 and 10:00. (no under-arm body odour). We have beend doing circadian for about a week, mainly because she wakes up late, so her ACTH-secretion is not controlled between 4AM and 9AM (during the peak times). Could you give your timing and percentages of medication, maybe I will discuss it with our endo if it could be used for our daughter. (now that her last Cortef does is given in the afternoon, she is missing her "bed" medication...)

ps.: Moeller is an endocrinologist, who suggested that CAH patients should take medication following the natural circadian rythm of 17-ohp level. In fact I did not manage to find the full-text article, so I do not know about the research method, sampling, etc. He suggest the folowing: 03:00 30%; 07:00 30%; 12:30 25%; 17:30 15%
