If he actually has CAH he needs to be on his meds. He could survive without them as long as there is no major shocks to his system but a major illness or injury could be fatal. As for fertility, sperm will not mature if his hormones are badly out of balance although this could be corrected by being back on meds. He is playing Russian roulett!
