Bonnie; This is a tough question to answer. I think the good news is that his bone age is good so once his growth hormone kicks in at least his bones have the "space" to grow.

I cannot emphasize more that the steroid signalling pathways are too complex for anyone to undertsand. It is clear one dose does not FIT ALL. Since every child is unique we have lot of different clinical scenarios.

So your son may not be oversuppressed and with all likely he will grow as soon as his own growth hormone kicks in but there should be a plan for using synthetic Growth hormones if he continues to fall short. I think his endo should be able to tell you when the time is right for that.

I believe that we should as a CAH community push for research into the "pharmacogenetics" of the steroid pathway. What that means is researchers can look how different genes play a role in the response to steroids and thereby we can personalize a "Steroid regimen" for every child/adult with CAH.

Sincerely,

KP

KP
