Our endo told us the most obvious thing in a boy with CAH is the penis size...our son's was 2 standard deviations bigger than 'normal.' Just like the girls with genatalia issues from extra testosterone, the boys also have extra testosterone. And my son was also severly jaundice. The lethargy came later, but as funny as this may sound, penis size can be an easy indicator.
