You don't say if he has been taking meds or not, that is the most important thing. It is likely that his dosage may be incorrect, you don't say what age he is so it may be off by a little or a lot, or may still be absolutely fine. Either way, you are right to be concerned, and I'd try to get your great grandson to an endo as soon as you can.
He is clearly not acutely suffering from incorrect treatment as he would have been in and out of hospital had that been the case, but in the long term there can be problems caused with poor treatment relating to fertility and growth, and testicular adrenal rests, which are obviously best avoided.
